Personal checks

Only money orders and cashier’s checks will be accepted at that
time. Late checks will be returned if received past deadline and
registration will not be processed.
Returned (bounced) Checks
~ Service fee for each returned check is $30.00 and registration
will not be processed.. When a replacement cashier’s check or money
order is received, registration will be processed.
Lost Badges ~
Processing fee for replacing lost badges is $30.00. Fee is not
refundable if found.

Special Needs ~
Special needs requests are limited to authorized event activities
only. They will be accepted until
June 6, 2007.
Registrants are responsible for arranging special needs outside of
authorized event activities at their own expense.
Hotel Reservation ~
See page eight (8) for a hotel reservation form. Registrants are
responsible for making hotel reservations. For more information,
please visit the Kellogg Conference Hotel web site,

The Kellogg
Conference Hotel at Gallaudet University has NEWLY renovated
guestrooms. Each room offers a coffee maker with complimentary
coffee, iron and ironing boards, TTY (Tele Type Writer), two phones,
Sorenson VP-100 Videophone, a 27” TV and a hair dryer. The marble
top desk and ergonomically correct chair provide an ample workspace
area with FREE high-speed Internet access. The rate for all sleeping
accommodations: single, double, triple and quad is $119.00. Please
note these rates are subject to the following taxes, currently:
14.5% on guest rooms. Please be aware that Check-in time is 3:00 pm
and check-out time is 12:00 pm on the day of your departure.
